Submission of complaint/claim

The complaint can be filed in writing or via email [email protected]

In writing to the address of the company: Křižovnická 86/6, Staré Město, 110 00 Praha 1

In the complaint for individuals, you must specify:

  • name and surname
  • Date of Birth
  • Permanent Address
  • transaction code
  • Subject of complaint
  • Be sure to attach any supporting documents

For legal entities:

  • Name of the company
  • an identification number
  • company address
  • transaction code
  • Subject of complaint
  • Be sure to attach any supporting documents
